Local acquisition, centrally run
Google search and Local Services, Meta and Business Profile management structured per location, with shared creative and compliance, and spend allocated to where the demand is.
Fractional CMO for multi location practices
I'm Monica Hacker, a fractional CMO for multi location medical, dental and specialty practices. Local acquisition by location, a centralized intake funnel, tracking that respects HIPAA, and reporting per site, with one senior person accountable for the number.
Part of my healthcare practice. Based in New York and Miami.
The pattern
Here's the shape I see in most practice groups. A corporate marketing budget spread across locations by headcount, not by opportunity. Google Business Profiles in different states of neglect. Paid search run as one campaign with every location lumped in. A call center or front desks with no shared script, no speed to lead standard and no visibility into which location is losing patients between the click and the appointment.
The economics of a practice group are local. Each location has its own demand, its own competition, its own reviews and its own intake bottleneck. Growth comes from treating them that way, with a central system for creative, tracking and follow up, and location level decisions about spend.
So I start with a scorecard. Cost per booked and shown patient by location and service line, where each intake funnel leaks, and which locations are starved while others are saturated. Then budget, creative and the intake process get rebuilt around it.
What I run
01
Google search and Local Services, Meta and Business Profile management structured per location, with shared creative and compliance, and spend allocated to where the demand is.
02
Online scheduling, call handling standards, speed to lead and follow up that work the same way everywhere, so a lead in one location isn't worth less than a lead in another.
03
Conversion tracking designed so no protected health information moves where it shouldn't, and a dashboard that shows leadership cost per shown patient by location, not just cost per lead.
Who this is for
A medical, dental, dermatology, physical therapy, behavioral health or specialty group with three to thirty locations, real media spend, and a marketing team or agency producing leads without location level accountability.
Often there's a private equity or operator team behind the group that wants marketing to report like the rest of the business. That's the reporting I build.
This sits inside my healthcare practice. Running a single aesthetic clinic instead? See fractional CMO for med spas.
Where to start
A location by location read on patient acquisition, the intake funnel, tracking and where budget is being wasted, delivered in 7 to 10 days with a recorded walkthrough.
What I look at first
Credited toward your first month if you move to a retainer.
Fair questions
Owns patient acquisition across every location: local paid and search by site, one intake standard, HIPAA aware tracking, and reporting that shows cost per shown patient by location. Leads the in house team or agency and answers for the number.
From $8,500 a month for about 10 to 12 hours a week, 4 month minimum, review at month 3. Larger groups usually need more hours, and I price that as a monthly retainer with the time commitment stated. A Growth Audit is $1,500 and credited toward your first month.
By designing measurement so the ad platforms see that a conversion happened and never what it was for or who it was. Server side setup, consent handling and a clear line on what data moves where. Your compliance team signs off. I make it work.
Usually no. Most groups have an agency running media well enough. What's missing is someone senior who sets location level priorities, holds the agency to a number and fixes the intake side. That's me.
Yes. Dental groups have the same shape: local demand, front desk intake, and a marketing budget that isn't allocated by opportunity. The playbook is the same and the reporting is what changes.
I take on a few new clients each quarter. If you're running a practice group that's ready to grow, let's talk.
